Better Futures

Better Futures is a new way of supporting care leavers, engaging with young people and their support networks, including case managers and care teams, early in their transition from care.

Working in collaboration with the Victorian Government funds MOIRA’s Your Journey team aims to engage earlier with care leavers, supporting them to have an active voice in their transition planning, and providing individualised supports.

You and your Better Futures worker can work together to decide what your goals are and how we can support you to achieve them.

A Better Futures worker can support you to:

  • Strengthen your independent living skills such as cooking and budgeting
  • Explore and access education or employment opportunities
  • Connect with different groups in your community
  • Access funding to support your goals towards independence
  • Better Futures workers can also meet with your care team, families/carers or school support group and work together to support you.

The Community Connector program is committed to encouraging young people to achieve their unique goals, reduce isolation and support the development of meaningful and sustainable community relations. We aspire to achieve this by working together with the community and creating new opportunities for young people to live rewarding lives as valued members of society.

By creating robust community opportunities, young people with a care experience can be supported to increase their sense of belonging, chase their ambitions and realise their potential as individuals.
