Leaving Care and Post Care

Our leaving care and post care programs are here to help young people become more independent as they leave state care.

This program aims to achieve better outcomes for young people leaving state care by assisting in the development of life skills, providing links to education, employment and training options, and preventing homelessness through the creation of pathways to sustainable, long-term housing options.

Who is eligible?

Pathways is for young people over the age of 16 who are in state care. This service is designed to help during the transition to independence when they exit care at age 18.

This service helps young people while they are transitioning to independent living after they have left state care. It helps young people achieve and maintain independence and stability by providing them with information, advice, brokerage and support. MOIRA runs this service from our Hampton East office, serving the entire southern area of Melbourne.

Do you need help with leaving care now?

Please contact MOIRA Youth Services. If you live outside the southern area of Melbourne, please phone the Leaving Care Helpline on 1300 532 846 or visit the Young Care Leavers website.
